1. If you have any recent relevant pathology testing results, please submit them here. Once submitted, you will be redirected back to this page to continue booking.

1) To make an appointment, scroll down until you see the appointment booking form.

2) Select your booking date and time.

3) You will be prompted to fill out a quick health form.
